Arbors at Gallipolis

The Arbors at Gallipolis Nursing Home is a 99-bed facility located at 170 Pinecrest Drive in Gallipolis, Ohio. This nursing home is part of the Arbors at Ohio chain of nursing homes. There are 16 nursing homes which are part of the Arbors of Ohio chain on the list of Special Focus Facilities and other nursing homes with significant issues with resident care.

The Arbors at Gallipolis nursing home received a federal fine of $24,990.00 on April 21, 2025. It received a federal fine of $32,273.00 on July 5, 2023.

    This nursing home has had 21 total deficiencies documented across recent inspection reports. It has had deficiencies relative to infection rates. It has received deficiencies relative to improper drug labeling and storage, failure to provide appropriate pressure ulcer care, failure to ensure areas are free from accident hazards, failure to provide trauma-informed culturally competent care, and issues with binding arbitration agreement disclosure.

    As a result of a July 2023 inspection, this nursing home received eight deficiencies with a fine of $32,273.00 including actual harm for failure to ensure that areas of the nursing home were free from accident hazards, infection control failures, failure to protect residents from abuse and neglect, and failure to provide enough food and/or fluids to the residents.

    The Dickson Firm has recently been involved in a lawsuit against the Arbors at Gallipolis Nursing Home. We filed a wrongful death lawsuit on behalf of the family of James H. Smith. Mr. Smith choked to death at the Arbors of Gallipolis Nursing Home. He was not provided with appropriate supervision while eating. He was not provided with proper food.

    This nursing home has been cited for abuse by CMS and has had five substantiated complaints in the last 3 years that resulted in a citation, plus one facility incident resulting in a citation.

    One reviewer on Caring.com described it as the worst nursing home she has ever seen, alleging dirty conditions, incorrect medication administration, and residents left in bed for multiple days.

    This is another facility that is owned by ARK OPCO GROUP, LLC. According to Medicare.com, Craig Flashner, Noble Healthcare Management, LLC, Yitzchok Perlstein, and Prestige Administrative Services, LLC has operational and managerial control of this nursing home since July 1, 2015, except for Prestige Administrative Services, LLC, which has had operational and managerial control since January 1, 2025.

    Given our direct involvement with this nursing home in the James Smith case as well as the other citations and deficiencies reported, we have grave concerns about the care being provided at this nursing home. Given the fact that the nursing home has had issues with failure to provide appropriate pressure ulcer care and failure to ensure areas are free from accident hazards, we have grave concerns that this nursing home is adequately staffed.

    Very often, issues with wound care are the result of understaffing. When a resident cannot turn and reposition themselves, the staff needs to turn and reposition them every 2 hours when they are in bed and often every hour when they are in a chair. If they begin to develop skin breakdown, then the nursing home must turn them more often. As they begin to develop skin breakdown, the nursing home staff must keep pressure off that area so that it can heal.

    Nursing homes are legally obligated to provide their residents with adequate supervision and to keep the nursing home free from hazards to prevent accidents. This nursing home has been cited for improper care leading to skin breakdown and also for violating the federal regulations that require them to keep their area free of accident hazards.

    Given our direct involvement with a case against this nursing home and the other complaints, issues, and citations, we strongly recommend that if you have a loved one at this nursing home, you consider moving them to another nursing home. If you are looking for a nursing home in the Gallipolis area, we strongly recommend that you consider other nursing homes.
