The Muskingum Skilled Nursing & Rehabilitation nursing home has an overall rating of two (2) out of five (5) stars, which is below average.
It also has a rating of one (1) out of five (5) stars for health inspections, which is much below average.
This nursing home received a large federal fine of $104,348 on December 4, 2023. On that same date, Medicare refused to make payments relative to the care being provided at the Muskingum Skilled Nursing & Rehabilitation nursing home.
The nursing home received two other federal fines in the amounts of $34,938 on July 15, 2024, and $59,440 on August 13, 2025.
Since 2019, the Muskingum Skilled Nursing & Rehabilitation nursing home has been cited for 63 deficiencies, including failure to provide appropriate pain management and failure to implement an infection prevention and control program.
On December 4, 2023, it was determined that the Muskingum Skilled Nursing & Rehabilitation nursing home caused actual harm to its residents by failing to protect them from abuse.
One resident experienced harm when a state tested nurse aide (STNA) took humiliating photos of the resident wearing a soiled incontinence garment without their consent. The STNA then showed the photos to the Administrator and other staff.
On August 13, 2025, it was determined that the nursing home put the health and safety of its residents in immediate jeopardy by failing to respond to allegations that a licensed practical nurse had physically assaulted two residents.
According to family members of residents, residents are frequently left wet and dirty without enough staff to provide for their most basic needs. Residents are also deprived of warm food, clean linens, and a clean environment to reside in.
We strongly recommend that anyone who has a loved one at this nursing home move them to a better facility. If anyone is considering putting their loved one in this nursing home, we strongly recommend that you consider other options.
