Blossom Care Center of Cuyahoga Falls is a 122 bed facility located at 300 East Bath Road, in Cuyahoga Falls, Ohio 44223. On www.medicare.gov, it is still listed as Continuing Healthcare of Cuyahoga Falls. However, the nursing home recently changed its name to Blossom Care Center of Cuyahoga Falls.
According to www.medicare.gov, this facility is part of the Paradigm Healthcare chain of nursing homes. It’s legal business name is Continuing Healthcare of Cuyahoga Falls, LLC. It is owned by Cuyahoga Operator Holdco LLC. This nursing home is not currently rated, due to a history of serious quality issues. This nursing home was part of a special focus facility program, and has been for 25 months. This nursing home received a federal fine of $18,887.00 on July 18, 2024. It received a federal fine of $7,878.00 on December 23, 2024. It received a federal fine of $124,098.00 on April 1, 2025. It received a federal fine of $39,520.00 on November 19, 2025.
CMS has designated this facility as a Special Focus Facility, based on a history of serious quality issues. As a result, it does not have a five star rating. The facility was sold by Continuing Healthcare Solutions in 2022. In May of 2024 the name of the nursing home was changed to Blossom Care Center of Cuyahoga Falls. The nursing home remains on the Special Focus Facility list, and as of early 2026, this facility has been on the Special Focus Facility list for more than twenty (20) months.
State Nursing Home Surveyors found the facility caused immediate jeopardy to residents in eight (8) separate deficiencies from July 13, 2018 to March 1, 2023, and caused actual harm to residents in twelve (12) separate written deficiencies from November 26, 2013 to March 1, 2023. The facility has received the worst possible CMS ratings across multiple categories, over extended periods of time. It had the worst overall rating from October of 2019 through March of 2024, for a period of 46 months. It had the worst health inspection rating for 52 months, from April of 2019 through March of 2024. And it had the worst staffing rating for a period of 45 months, from April of 2018 through March of 2024.
The facility has paid eight federal fines over a 3 year period, totaling $311,303.00.
11.3% of short term rehabilitation residents developed a serious infection requiring hospitalization, compared to the Ohio average of 7½%, and the national average of 7.7%.
Recent inspection reports document multiple deficiencies, including staff neglect of a resident that was neither reported, investigated, nor corrected and repeated falls, causing actual harm to a resident without an inadequate investigation. In December of 2024, a 92 page inspection report was issued citing numerous deficiencies, including failure to provide sufficient nursing staffing. Documented violations include failures to protect residence from abuse, neglect, and exploitation, failure to timely report and investigate suspected abuse and neglect, failure to prevent and control infections, failure to prevent and treat pressure ulcers, failure to maintain medication error rates below 5%, failure to implement gradual dose reductions before initiating psychotropic medications, failure to ensure residents are free from significant medication errors, and failure to provide a safe, clean, and home like environment.
A female resident with dementia was sexually assaulted by a nurses' aid at the facility. The employee pled guilty in November of 2018 to two felony counts of sexual battery, and a felony count of gross sexual imposition, and was sentenced to 5 years in the Lorain Correctional Institute. Recurrent reviews describe chronic understaffing, and high staff turnover, as a central theme, with reports of neglected call buttons, extended delays in assistance, missed medications, and failures to manage essential respiratory equipment. Serious safety incidents include resident falls, choking events, and residents being assaulted by other patients, not adequately managed.
A newly released Ohio Department of Aging Report, found that Ohio's direct care staffing levels fell below the national average, with certified nurse aid staffing below that of all neighboring states, except one, and turnover rates for total nursing staffing and registered nurses above the national average, and higher than all neighboring states.
