The Arbors at Minerva Nursing Home is a nursing home located at 400 Carolyn Court in Minerva, Ohio.

It is a nursing home with 92 beds.  According to Medicare, it cares for an average of about 70 residents per day.

This nursing home is owned by ARK OPCO GROUP, LLC, which has owned 100% of it since July 1, 2015.  Medicare lists Noble Healthcare Management, LLC and Prestige Administrative Services, LLC as having managerial control of this nursing home.  It has an overall rating of 4 out of 5 stars.

On January 23, 2025, this nursing home was cited for failing to adequately monitor and provide timely and necessary care and treatment to a resident following a change in condition.  Inspectors found that this failure caused actual harm.  According to the inspection report, the resident had been given medication for nausea and vomiting twice on December 26, 2024.  At 1:48 A.M. on December 27, 2024, the resident vomited brown-colored material and had a decline in her vital signs.  Her blood pressure was 94/52, her heart rate was 120, and her oxygen saturation was 90%.  She complained of aching all over and was given Tylenol.  When a Licensed Practical Nurse later went into the resident's room to check on her, she said she did not feel right.  She was breathing short, rapid breaths, she was pale, cold and moist, her blood pressure could not be read, and her oxygen saturation was 76%.  The physician was then notified and ordered that she be sent to the emergency room.  Before EMS arrived, the resident went into cardiac arrest.  CPR was performed for approximately 27 minutes without success, and the resident was pronounced dead at 5:16 A.M.  The nursing home was also cited for failing to notify the resident's physician of a significant change in her condition.

On April 3, 2023, this nursing home was cited for failing to keep the nursing home free from accident hazards and to provide adequate supervision to prevent accidents.  Inspectors found that this failure caused actual harm to a resident.

On October 24, 2023 and again on June 25, 2024, in response to complaints, this nursing home was cited for failing to protect residents from abuse and neglect.

On December 18, 2023 and again on August 22, 2024, this nursing home was cited for failing to provide appropriate pressure ulcer care and to prevent new pressure ulcers from developing.

On July 16, 2026, in response to a complaint, this nursing home was cited for failing to ensure that each resident's drug regimen is free from unnecessary drugs, failing to provide appropriate treatment and care according to orders and the resident's preferences and goals, and failing to provide and implement an infection prevention and control program, among other deficiencies.

In total, Medicare's records show thirty-six (36) deficiencies at this nursing home.

There are sixteen (16) nursing homes in Ohio known as “Arbors”.  Medicare identifies all sixteen of them as part of the same chain, “Arbors at Ohio”.  Of these sixteen (16) nursing homes, four (4) of them have been flagged by Medicare because they have been cited for abuse.  Seven (7) of these sixteen (16) nursing homes have a 2 out of 5 star rating.  The average health inspection rating for the chain is 2.4 out of 5 stars.

In June 2026, Signal Ohio reported that since January 1, 2024, at least eleven (11) plaintiffs have filed lawsuits accusing Arbors nursing homes of negligence or medical errors that caused residents' deaths.  Signal Ohio also reported that federal inspections found that poor care at the Arbors at Minerva, the Arbors at Milford, and the Arbors at Stow contributed to residents' deaths.  The Lucas County Coroner ruled that the July 2024 death of a resident of the Arbors at Oregon was a homicide, caused by a Stage 4 bedsore that resulted from medical neglect.

If someone you love is in a nursing home, and you are not confident that they are receiving appropriate care, it is imperative that you move them to a better nursing home.  No amount of family involvement or frequent visiting can make up for a poor nursing home that is improperly staffed or otherwise not providing appropriate care to its residents.  We understand that moving an elderly nursing home resident, particularly one that's confused or demented, to another facility can be very traumatic.  However, if your resident is not getting appropriate care, then they are at risk, and you must consider moving them to a better nursing home.

We handle cases throughout the State of Ohio.  And we see multiple nursing homes that consistently provide improper care.  They do not staff their facilities properly.  They do not hire qualified people.  And their residents are consistently neglected and abused.
